What is Managed Long-Term Care?
Managed Long-Term Care (MLTC) is a Medicaid plan for people who need ongoing health services and support. It lets you get affordable health care services at home and in your community. This way you can stay independent and live at home. If you are disabled or have an ongoing medical condition, you might be eligible for MLTC.
Aetna Better Health® is a managed care organization (MCO) that takes part in the New York State Medicaid program. This means that we can provide you with your Medicaid MLTC plan.

If you qualify, you’ll be covered for benefits and services like:
Home health care
Care management
Social day care services
Rides to medical visits
Medical equipment
Dental care
Vision care
Who can get MLTC?
Who can get MLTC?
MLTC is for seniors or other adults who need regular care or help with daily living. This could be due to a disability or long-lasting health problem.
To get MLTC services, you must have Medicaid and:
- Be 21 or older
- Need a nursing home level of care
- Be able to live at home without any safety issues
- Need at least one of these community-based long-term services and supports:
- Home nursing services
- Therapies in the home
- Home health aide services
- Personal care services in the home
- Adult day health care
- Private duty nursing
- Personal assistance services
- Live in one of our Plan's service areas:
- Manhattan
- Brooklyn
- Queens
- Bronx
- Nassau County
- Suffolk County
