Your Provider Relations team
After you join our network, we’ll assign you a network consultant. They’ll be your primary point of contact with us, sharing important information with you. They’ll also help you with any admin or operational concerns, like tracking member information and updates, finding forms and reviewing claims or remittance advice.
Your network consultant will:
- Share our administrative policies and procedures
- Communicate changes and updates that help you in the efficient administration of our plans
- Advocate on your behalf to resolve issues
- Manage changes to your demographic information
Our network consultants are based in the communities they serve, fostering personal relationships and a high level of responsiveness. They engage with providers in a variety of ways to provide proactive, prompt and collaborative communication.

How to join our network
1. Let us know
Just email Provider Relations and let us know that you’d like to join. Or you can call us at 1-855-456-9126 (TTY: 711). We’ll follow up with the application form you need to get started.
2. Fill out your application
When you get your application form, just fill it out and email it back to us.
3. Complete your contract
If your application is accepted, we’ll offer you a contract and send it to you. Just complete your contract and follow the instructions included to return it.
